Schedule & Territory

• Minimum Commitment: Flexible availability for at least 2–3 days per week.
• Territory Coverage: Focused exclusively within the San Fernando Valley. You will be matched with patients in designated local clusters close to your home base.

Key Clinical Responsibilities

• Conduct in-home primary care evaluations, routine wellness checks, and chronic disease follow-ups.
• Perform comprehensive wound care assessments, routine dressings, and clinical treatment protocols.
• Review mobile lab/diagnostic results and prescribe or adjust medications accordingly.
• Complete concise, point-of-care charting via our mobile EMR system.
• Coordinate with clinical support staff and specialists for patient continuity of care.

Qualifications & Requirements

• Licensure: Active, unrestricted California license as a Nurse Practitioner (NP) or Physician Assistant (PA).
• DEA Registration: Must possess an active, unencumbered DEA license alongside a CA furnishing/prescribing license.
• Language High Priority: Fluency in Armenian or Spanish is strongly preferred.
• Mobility: Valid CA Driver’s License, reliable transportation, and active automobile insurance.
• Experience: Background in In-home Primary Care, Home Health, or Wound Care is preferred, but motivated and licensed clinicians willing to be trained on mobile protocols are encouraged to apply.
