Practice at the top of your license in advanced wound care
TruMend Wound Care delivers advanced bedside wound management across private homes, assisted living and memory care communities, and skilled nursing facilities. This is focused, procedure-driven, outcomes-based specialty practice, not primary care. We are building per-visit provider coverage in the East Valley and adding a Nurse Practitioner or Physician Assistant who wants real autonomy, a procedural focus, and flexible per-visit work.
How this role works
This is a transactional, per-visit role. You accept visits in your area, see the patient where they live, perform the indicated wound procedure, document the encounter, and get paid a flat fee per visit. You build your own schedule and manage your own route, with the expectation that assigned patients are seen in a timely manner consistent with their plan of care. There are no fixed shifts and no minimum hours, but wound patients need consistent, on-time follow-up, so responsiveness and reliable turnaround on accepted visits matter.
Why this role stands apart
● Per-visit pay with a very competitive flat fee for each completed encounter
● A meaningful procedural component across advanced wound modalities
● Full practice autonomy in the field, backed by collaborative physician support
● Mileage covered for field travel
● Growth opportunity inside a premium, rapidly expanding specialty platform
Procedures and modalities
You will perform bedside wound procedures, including:
● Sharp debridement
● Collagen wraps and advanced dressing selection
● Ultra-MIST (non-contact low-frequency ultrasound) therapy
● Skin substitute graft application (cellular and tissue-based products)
● Wound culture collection and biologic modality selection
● Other advanced wound modalities as indicated
Position overview
The provider delivers comprehensive wound assessment, procedural intervention, and care management where patients live. You function as the clinical lead in the field, directing evidence-based wound protocols and coordinating with facilities, home health partners, and referring providers.
Core responsibilities
● Perform comprehensive wound evaluations and build individualized treatment plans
● Conduct bedside procedures as listed above
● Manage complex wounds, including pressure injuries (Stage I to IV), diabetic foot ulcers, surgical wounds, and venous and arterial ulcers
● Prescribe wound-related medications and adjunct therapies
● Coordinate care with referral partners and collaborating physician support
● Document consistent with regulatory and payer requirements
● Educate facility staff, patients, and families on prevention and care plans
● Monitor healing progress and adjust interventions accordingly
Who this is for
This role fits an NP or PA who is confident performing bedside procedures, comfortable practicing independently in the field, fluent in payer documentation requirements, and looking for flexible per-visit work. It suits someone who wants a focused specialty over broad primary care.
Who this is not for
This is not a fit for candidates who prefer clinic-based primary care, are uncomfortable with procedures, require constant on-site supervision, or want a fixed hourly schedule.
Required qualifications
● Active Arizona NP or PA license in good standing
● National board certification (FNP, AGNP, NCCPA/PA-C, or applicable certification)
● DEA registration, or the ability to obtain one
● 3+ years of clinical experience
● Experience in home health, SNF, or post-acute environments
● Reliable transportation and willingness to travel within the East Valley
● Comfort with independent, field-based clinical decision-making
Preferred qualifications
● Wound care certification (WCC, CWCN, CWS, CWSP, or equivalent)
● Procedural wound care and debridement experience
● Experience with Ultra-MIST, skin subst…
