About the Role
TrestleCare Medical Group PLLC is seeking an experienced, compassionate Nurse Practitioner to provide virtual primary care and chronic condition management for New York patients, primarily Medicare beneficiaries with complex health needs.
Our model combines telehealth visits, Chronic Care Management (CCM), Remote Patient Monitoring (RPM), and collaboration with care coordinators and home-based and community providers. Together, we help patients manage chronic conditions, access needed services, and remain supported at home.
The ideal candidate brings sound clinical judgment, a warm communication style, and an interest in helping develop a growing medical practice. You should enjoy connecting with patients and caregivers virtually and working with a multidisciplinary team to turn care plans into practical, ongoing support.
Responsibilities
- Conduct comprehensive virtual assessments, evaluate symptoms, diagnose conditions, and develop appropriate treatment plans within your authorized scope of practice.
- Provide primary care and ongoing management of chronic conditions for adults, including older adults and patients with multiple medical conditions.
- Review medical histories, reconcile medications, and identify preventive care needs and gaps in care.
- Develop and update individualized care plans in collaboration with patients, caregivers, and the care team.
- Order appropriate diagnostic tests, interpret results, coordinate referrals, and arrange follow-up.
- Prescribe medications within your authorized scope and in accordance with applicable New York State and federal requirements, including requirements governing telehealth and controlled substances.
- Provide clinical direction for care coordination and care management activities performed by designated team members.
- Review care coordinator reports and remote monitoring data as clinically appropriate, respond to escalations, and adjust treatment plans when indicated.
- Determine when patients require in-person assessment, urgent evaluation, or emergency care, and coordinate appropriate next steps.
- Educate patients and caregivers about diagnoses, medications, treatment plans, and self-management.
- Complete timely, accurate documentation in the electronic health record, supporting medical necessity, services performed, and applicable billing requirements.
- Follow applicable privacy, security, telehealth, and patient consent requirements.
- Contribute clinical input to the development and improvement of care workflows, protocols, and quality initiatives.
Required Qualifications
- At least two years of clinical experience practicing as a Nurse Practitioner in primary care, geriatrics, or a related setting.
