Telehealth Nurse Practitioner / Physician Assistant – CA, NY & NJ Licensed

JOB SUMMARY

Renova Complete Care is seeking an experienced Nurse Practitioner or Physician Assistant for a remote telehealth role evaluating patients age 65 and older for enrollment in chronic care management and related care programs.

Active provider licenses in California, New York, and New Jersey are required, along with appropriate prescribing authority. Additional state licenses are a significant advantage.

You will conduct clinical assessments, review medical histories and medications, determine program eligibility, prescribe medications when clinically appropriate, and prepare individualized care plans and documentation. Our care management team handles ongoing CCM calls and routine monthly care management activities.

The ideal candidate has internal medicine, adult primary care, or geriatrics experience, strong telehealth skills, and excellent communication with older adults and caregivers. CCM, RPM, or BHI experience is a plus.

COMPENSATION

$40–$50 per completed telehealth evaluation, including record review, the patient visit, clinically appropriate prescribing, care plan preparation, and finalized documentation.

The displayed $60–$75 hourly range is an estimated equivalent based on approximately 40 minutes of total work per evaluation. Payment is per completed visit, not hourly; actual hourly earnings vary with time required and completed visit volume. Payment does not depend on whether the patient qualifies for or chooses to enroll.

Expected commitment: Approximately 5–15 hours per week.

Pay: $60.00 - $75.00 per hour

Expected hours: 5.0 – 15.0 per week
